I am Donna, I work as a Hospital to Home Caseworker for Care and Repair in Torfaen.
Care and Repair are an older person's charity, primarily supporting people over the age of 65. We help private home owners and private tenants to remain independent and improve the safety of their properties through assistance and advice.
We offer a 'Healthy Home Check' which means visiting the property to identify and discuss areas of concern. We conduct fire safety, falls risk assessments and help maximise income through benefits checks. Utilising minor aids and adaptations grants, we can help pay for hand rails, grab rails, keysafes and many other useful aids to improve safety and reduce falls risks.
We also have a register of approved local contractors who can be utilised for larger private works, and technical officers who can conduct property surveys and make recommendations.
My role is specific to supporting people who are being discharged from hospital, however, we encourage anyone living within Torfaen with any enquires or property related concerns to call our friendly office staff on 01495 745948, we are happy to help or signpost for further support.
