We would like someone who can...

- Be person-centred and allow the person to shape the support they wish to receive;
- Establish appropriate boundaries with a client.
- Comply with Age Cymru values, policies and procedures.
- Respond to requests and requirements by your supervisor at Age Cymru.

About the opportunity
You'll support older people and carers in your local community to have their voices and wishes heard and to achieve their personal aspirations and goals. Within this role you'll support people with their issues or concerns and to ultimately empower them to be able to resolve similar issues themselves in the future. Together you'll develop a personal action plan and work to achieve their aspirations.
Duties and responsibilities:
- To support clients either via the telephone or face-to-face with issues, challenges and barriers impacting their day-to-day lives.
- Develop action plans with the people you are working with and work together to achieve their desired outcomes.
- Provide practical support to the client as needed (e.g., helping someone to write a letter, prepare for a meeting, researching options and services available to support the individual);
- Keep written records of your meetings that comply with GDPR standards and meet regularly with your supervisor to discuss your case and reflect on your practice. What activities will they do?

Hoffwn rywun sydd yn…

- Medru deall a gwrando ar yr unigolyn rydych chi’n ei gefnogi.  Rydych chi’n deall eu hanghenion a’u nodau unigryw.

- Helpu pobl i ddeall a mynychu’r cyfleoedd a’r wybodaeth sydd ar gael.  Rydych chi’n medru eu grymuso i wneud dewision a phenderfyniadau gwybodus.

- Datblygu cynlluniau gweithredu gyda’r bobl rydych chi’n gweithio gyda nhw, a chyd-weithio i gyrraedd eu canlyniadau dymunol.

- Rydych chi’n ffocysu ar yr unigolyn, a’u galluogi i ddewis y gefnogaeth yr hoffent ei dderbyn;

- Sefydlu ffiniau addas gyda chleient.

- Cydymffurfio gyda gwerthoedd, polisïau a gweithdrefnau Age Cymru.

- Ateb ceisiadau ac anghenion eich arolygwr yn Age Cymru.

- Rydych chi’n breswylydd yng Nghymru.

Gwybodaeth am y cyfle
Byddwch chi’n cefnogi pobl hŷn yn eich cymuned leol. Byddwch chi’n sicrhau bod eu lleisiau a’u dymuniadau’n cael eu clywed, a’u cynorthwyo i gyflawni eu huchelgeisiau a’u nodau personol. Fel rhan o’r rôl byddwch chi’n cefnogi pobl gyda’u problemau a’u pryderon. Byddwch chi’n eu grymuso i ddatrys problemau tebyg eu hun yn y dyfodol. Byddwch chi’n datblygu cynllun gweithredu personol gyda’ch gilydd, ac yn gweithio i gyflawni eu huchelgeisiau.
Dyletswyddau a chyfrifoldebau:
- Cefnogi cleientiaid dros y ffôn neu wyneb yn wyneb gyda phroblemau, heriau a rhwystrau sydd yn effeithio ar eu bywydau bob dydd.
- Datblygu cynlluniau gweithredu gyda’r bobl rydych chi’n gweithio â nhw. Cyd-weithio i gyflawni eu nodau.
- Darparu cefnogaeth ymarferol sydd ei angen ar y cleient (er enghraifft helpu rhywun i ysgrifennu llythyr, paratoi ar gyfer cyfarfod, gwneud gwaith ymchwil am ba opsiynau a gwasanaethau sydd ar gael i gefnogi’r unigolyn);
- Cadw cofnodion ysgrifenedig o gyfarfodydd sydd yn cydymffurfio â’r Rheoliad Cyffredinol ar Ddiogelu Data. Cyfarfod gyda’ch goruchwyliwr yn gyson i drafod eich achos a thrafod eich gwaith. Pa weithgareddau fyddant yn eu gwneud?
