About the opportunity
You'll support older people and carers in your local community to have their voices and wishes heard and to achieve their personal aspirations and goals. Within this role you'll support people with their issues or concerns and to ultimately empower them to be able to resolve similar issues themselves in the future. Together you'll develop a personal action plan and work to achieve their aspirations.
Duties and responsibilities:
- To support clients either via the telephone or face-to-face with issues, challenges and barriers impacting their day-to-day lives.
- Develop action plans with the people you are working with and work together to achieve their desired outcomes.
- Provide practical support to the client as needed (e.g., helping someone to write a letter, prepare for a meeting, researching options and services available to support the individual);
- Keep written records of your meetings that comply with GDPR standards and meet regularly with your supervisor to discuss your case and reflect on your practice. What activities will they do?
